1. The plan is valid for 12 months from the date of enrollment.

3. The plan is non-transferable and is limited to one person per enrollment.
4. Follow-up visits and other services included in the plan must be scheduled within the plan’s validity period.
5. After the 12-month period, the plan can be renewed to continue availing of the benefits.
6. Additional services beyond the plan’s coverage will be billed at standard rates.
